[OCCAID] 2001:470:112::/48 and 3ffe:4010:a00e::/48 now removed

The above two legacy blocks, which we warned users to renumber out of for
two years now, are finally unrouted on the OCCAID network.

We have sent enough notifications to everybody through authorized channels that
users must renumber to 3ffe:401d::/32 since Dec 2003. Thankfully many of them
did renumber, but a few (5 sites actually) did not even today.

So if you are one of those 5 sites still on the two /48's, please wake up.
